Program Overview
Franciscan Health is teaming up with the Indiana State Department of Health on diet options as well as chronic disease management education and support. Program participants will receive monthly voucher supplements for fresh fruits and vegetables!
Program Qualifications
• Have a type 1 or type 2 diabetes diagnosis or some type of chronic disease
• Screen positive for moderate to high food insecurity
• Complete a 5-week Cooking Matters course
