Program Overview
Franciscan Health has partnered with the Indy Hunger Network for a nutrition education collaboration for chronic disease management education and support. Program participants will receive 6 monthly voucher supplements for fresh fruits and vegetables!
Program Qualifications
• Have a type 1 or type 2 diabetes diagnosis or any type of chronic disease
• Screen positive for moderate to high food insecurity
• Complete a 6-week Prod Rx/Cooking Matters course
